  • Social policy and procedures (Anti-corruption, harassment policy, discrimination policy etc)
  • MSDS
  • Documentation of all trainings to workers   (Last conducted-Fire drill record, fire fighting training, first aid training and ppe training)
  • Minutes of Meetings (Participation committee last meeting)
  • Wage List (Salary sheet of 3 workers)
  • Working Time Record (Time card of same 3 workers)
  • Copies of official documents (Factory license, trade license and incorporation certificate)     
  • Inspection Report of Firefighting Equipments (0ne fire extinguisher check list)
  • Inspection reports dangerous machines (Daily check list of Boiler and generator-Current page + Cover Page)
  • Personal Data File (Any one worker’s personal file)
  • Qualification proof (Competency certificate of Boiler operator and Electrician) note: No ABC permit and no boiler op.
  • Complaint mechanism (Grievance policy + Grievance register-Current page + Cover Page)                                                                     
  • Valid business licenses (Fire license, EPB, IRC, ERC, BERC and Boiler)
  • List of valid certificate (Membership certificate)
  • Evidence of updated contribution to social insurance funds (Group insurance certificate)
  • Inspection reports on water portability (Water test report)
  • Inspection Reports Concerning Health (Air emission, noise, temperature, light test)
  • Documentation of the legal minimum wages (Minimum wage policy)
  • Management Representative (Office order for responsible person to implement BSCI CoC)
  • Employment contracts (One Security guard personal file) 
  • Documentation on All Benefits (One maternity personal file)
  • Documented record of the accidents (Accident register-Current page + Cover Page)    
  • Documented risk assessment (Risk assessment)      
  • Medical Personnel (Appointment letter and competency certificate of Doctor and Nurse)             
  • Pay slips (One pay Slip copy)
